attending, or paying for services, the client agrees to be bound by the terms outlined herein.

1. Services Provided
Elective, concierge, and wellness-focused medical services that are not billed to insurance.

2. Client Responsibilities
Accurate disclosure of medical history, timely payment, adherence to instructions.

3. Limitations of Liability
Outcomes may vary; no warranties expressed or implied. NeuLogics is not liable for subjective
dissatisfaction.

4. Jurisdiction
This agreement shall be governed by the laws of South Carolina.

5. Modifications
NeuLogics reserves the right to amend these terms at any time.

NeuLogics Wellness & Medical is committed to protecting the privacy of our patients. This policy
outlines how we collect, use, and safeguard personal information.

Name, contact information, health history, appointment records, and payment information.

2. Use of Data
Used solely for medical care, communication, payment processing, and compliance.

3. Protection Measures
Secure storage, access controls, and encryption measures in place.

4. Third-Party Sharing
No data shared without patient consent, except where legally required.

E-Commerce Policy

NeuLogics Wellness & Medical offers medical services, wellness consultations, and related
therapies through a private, cash-based model. All payments are securely processed via
authorized payment gateways, including Stripe.

1. Accepted Payment Methods:
We accept credit cards, debit cards, HSA/FSA cards (when approved), and direct ACH via
Stripe. Payments are processed securely using encryption and are subject to authorization
and approval.

2. Pricing:
All prices are listed in USD. Pricing may vary by treatment complexity, medical condition, or
changes in product cost. We reserve the right to adjust pricing at any time with or without
notice.

3. Billing Policy:
Payment is due at the time of service unless otherwise arranged. Deposits may be required
to schedule certain services or to hold appointment slots.

4. Refund Policy:
All sales are final. We do not offer refunds for medical consultations, services rendered, or
medications ordered through licensed compounding pharmacies. Exceptions may be made
in cases of error or cancellation with appropriate advance notice.

5. Appointment Cancellation:
Cancellations must be made at least 48 hours prior to a scheduled visit. Missed
appointments without proper notice may result in a forfeiture of your deposit or incur a
cancellation fee.

6. Security:
All payment data is encrypted and securely processed through Stripe, a PCI-compliant
payment processor. We do not store your payment details on our servers.

7. Communication and Disputes:
If you have any concerns regarding a charge, please contact us at info@neulogics.org prior
to filing a dispute. We are committed to resolving concerns quickly and fairly.

8. Shipping & Delivery Policy
If the shipment of any medication, supplement, or product from NeuLogics Wellness &
Medical is required, a $25 shipping and handling fee will apply. All items are shipped via
UPS 2-Day Delivery unless otherwise specified. We currently ship to all U.S. states. Once
shipped, you will receive a tracking number for your records. NeuLogics is not responsible
for delays caused by carrier services, weather, or other factors beyond our control.
